In accordance with 201 KAR 30:330

If information required or requested by the board, through application or otherwise, becomes inaccurate the registrant shall file with the board an amendment correcting that information within ten (10) business days. 

 

**Change Of Information - use for changes to address, phone number, fax number, or email ONLY.

 

**Do NOT fill out the Change of Information form for a change in Designation of Compliance Manager. You must fill out the Designation of Compliance Manager.pdfDesignation of Compliance Manager.pdf and complete a background check.  Instructions for completing the required background check are included with the Designation of Compliance Manager application.​​
