In accordance with 201 KAR 30:330, Section 1, (2):
If information required or requested by the board, through application or otherwise, becomes inaccurate the registrant shall file with the board an amendment correcting that information within ten (10) business days.

**Do NOT fill out the Change of Information form for a change in Designation of Compliance Manager. You must fill out the Designation of Compliance Manager Application and complete a background check. Instructions for completing the required background check are included with the Designation of Compliance Manager application.
